JOB TITLE: HEALTH NAVIGATOR

REPORT TO: CHIEF OFFICER OF GOVERNMENT & COMMUNITY AFFAIRS MANAGER

BASIC FUNCTION: Under the general supervision of the Clinic Manager or Office. Conduct public education activities to raise awareness about coverage options available under Medicaid, CHIP and the Marketplace.

REPRESENTATIVE DUTIES: The duties recorded below are intended to accurately represent the duties of the class and are not intended to cover every single duty of the job.

1. Must complete the certified Enrollment Counselor application and meet all requirements.

2. Conduct public education activities to raise awareness about coverage options available under Medicaid, CHIP and the Marketplace.

3. Help individuals understand and access affordability options.

4. Provide information and assistance in a fair, accurate, and impartial manner.

5. Provide information and assistance in a manner that is culturally and linguistically appropriate to diverse communities and accessible to individuals with disabilities.

6. Provide timely referrals to any applicable office of health insurance, and referrals to other resources, such as the toll-free Marketplace Call Center, or to other state or local entities that can more effectively serve that individual.

7. Will work to connect to local appropriate healthcare resources.

8. Assist individuals and families to apply and enroll into the health insurance Marketplace and/or into affordable health insurance coverage.

9. Conducts extensive outreach and health education and refers individuals to CSDCP services, and other community resources as appropriate.

10. Conducts health education or coaching activities with individual patients or groups as necessary.

11. Explains and distributes written materials regarding clinic sites and services.

12. Encourages patients to access primary care services, schedules primary care appointments as necessary.

13. Makes appropriate referrals for patients to other services and tracks patients follow through on referrals.

14. Administers evaluation or data collection forms as required.

15. Meets with supervisors regularly to report on progress orally and in writing s requested.

16. Maintains active communication with clinic staff as necessary.

17. Other duties as assigned by Supervisor.

EDUCATION AND EXPERIENCE: Any combination equivalent to: graduation from high school or GED equivalent. Community outreach experienced preferred. Must have and maintain a current BLS card.

KNOWLEDGE AND ABILITIES: Bicultural with a strong relationship and understanding of the Latino population in the Eastern Coachella Valley and surrounding areas. Strong interpersonal and communication skills desired. Bilingual in Spanish and English preferred. Must have a valid CA driver’s license, reliable personal transportation, proof of car insurance. Flexible schedule with the availability to work evenings and weekends. Ensures application and compliance of security protocols and polices such as HIPAA, Corporate Compliance, Confidentiality, etc.

Maintain privacy of all patient, employee and volunteer information and access such information only on a need to know basis for business purposes.
